Introduction. You are submitting a request for a loan pre-qualification or a loan referral form (hereinafter, a "Request") through Direct Money Source. For purposes of this consent, "DMS" means Direct Money Source. DMS can give you the benefit of our services online, by fax and by regular mail.

Electronic Communications. Any Disclosures related to your Request will be provided to you electronically through dmsfinancing.com. However, if you wish to obtain a paper copy of any of the Disclosures, you may provide us with a fax number or write to Direct Money Source,10880 Wilshire Blvd., Suite 530, Los Angeles, CA, 90024 with the details of your request. Paper copies will be provided to you at no charge.

Changes to Your Contact Information. You should keep us informed of any change in your electronic or mailing address. You may contact DMS Customer Service by telephone at 1888 677 7774 regarding any such changes.

YOUR ABILITY TO ACCESS DISCLOSURES. BY COMPLETING AND SUBMITTING YOUR REQUEST, YOU ACKNOWLEDGE THAT YOU CAN ACCESS THE DISCLOSURES IN THE DESIGNATED FORMATS DESCRIBED ABOVE.

CONSENT. BY COMPLETING AND SUBMITTING YOUR REQUEST, YOU CONSENT TO HAVING ALL DISCLOSURES PROVIDED OR MADE AVAILABLE TO YOU IN AGREED FORM AND TO DOING BUSINESS WITH DMS.
